In situations where you prefer to place an anonymous report in confidence, you are encouraged to use this channel. You are encouraged to submit reports relating to violations stated in our Code of Conduct, as well as asking for guidance related to policies and procedure and providing positive suggestions and stories.

How the Reporting Process Works

Complete the confidential reporting form with the details of your concern.

You will receive a unique Reference Number at the email address you provided.
↓
03 — Review
Your report is reviewed by authorized AWI personnel.
↓
04 — Follow Up
Use your Reference Number to check the status of your report.
Frequently Asked Questions
Can I report anonymously?
Yes, where anonymous reporting is available. You may choose not to provide identifying information.
What happens after I submit my report?
Your report will be recorded and assigned a unique Reference Number. Authorized personnel will review the concern.
Can I check my report later?
Yes. Use your Reference Number through the Follow Up section.
What if I don't know all the details?
Provide as much accurate information as you know. You can still submit a concern even if you don't have every detail.
Will AWI contact me?
If you provide contact information, an authorized member of the review team may contact you when appropriate.
Can I add information later?
Yes, through the Follow Up system where additional information is supported.
